Registered Nurse - Case Conference

Job Overview

Frontpoint Health is seeking compassionate and detail-oriented Registered Nurses (RNs) to join our US Operations team in a remote, full-time role. This position primarily involves participating in and coordinating case conference activities to support patient care planning and interdisciplinary collaboration in a home healthcare setting.

Key Responsibilities

  • Participate in interdisciplinary case conferences and contribute clinical insights.
  • Review patient records and assist in developing and updating care plans.
  • Collaborate with physicians, case managers, therapists, and other healthcare professionals to ensure continuity of care.
  • Document case conference discussions accurately and maintain patient records.
  • Coordinate with internal teams to support timely patient care activities.
  • Communicate effectively with healthcare providers and team members regarding patient status and care plans.
  • Promote high-quality patient care through effective care coordination and documentation.
  • Support additional clinical coordination tasks as assigned.
Qualifications
  • Bachelor's Degree, Diploma, or equivalent qualification in Nursing.
  • Active Registered Nurse (RN) license in India.
  • Experience in home healthcare, care coordination, utilization review, or case management is an advantage.
  • Strong clinical judgment, critical thinking, and documentation skills.
  • Excellent written and verbal English communication skills.
  • Comfortable working remotely during US shift hours.
  • Reliable laptop and stable high-speed internet connection.
Preferred Skills
  • Experience working with US healthcare processes is an added advantage.
  • Ability to work collaboratively within a multidisciplinary healthcare team.
  • Strong organizational and time management skills.
